About this test
This isn't a medical diagnosis. It's a way of organizing what I see every week in the clinic. There are lower-back pain patterns that repeat over and over, and almost no one explains them to you. By the end of the test you'll know which one you're in — and why the treatments you've tried haven't solved it.

Your pattern

Mechanical-postural pattern
Your pain mostly shows up with sustained postures — sitting, standing, getting up. This is common in spines that have lost segmental mobility: the body signals when you keep it still, not so much when it moves.
Effort-overload pattern
Your pain shows up with movement, training, or sudden gestures. It usually means the spine doesn't yet have the capacity to absorb that load — not that you should stop moving, but that it needs preparing to handle it.
Sensitized pain pattern
You've had background pain for a while that doesn't follow a clear logic and has already gone through several hands without resolving. When pain becomes chronic, the body often gets more sensitive than the tissue alone would justify — and that's where the approach changes.
Suspected structural pattern
You've been told about herniations, surgery, or something "structural". These findings often don't explain the pain you feel on their own — many spines with the same MRI findings don't hurt at all.
